This is a very clear explanation of the rather poor EAC menu labels. Can I suggest, though, that using the term "Index" for a duration can be confusing. Maybe you could include an actual (single WAV) cuesheet for the example, ie (if my maths is right)

TRACK 01

  INDEX 01 00:00:00

TRACK 02

  INDEX 00 01:00:00 - end of track 1, 1 minute long
  INDEX 01 01:02:00 - start of track 2 after 2 second gap

TRACK 03

  INDEX 00 03:02:00 - end of track 2, 2 minutes long
  INDEX 01 03:05:00 - start of track 3 after 3 second gap

TRACK 04

  INDEX 00 06:05:00 - end of track 3, 3 minutes long
  INDEX 01 06:09:00 - start of track 4 after 4 second gap
